Pathways PACE serves as both your Medicare provider and your long-term care provider. When you enroll in the Pathways PACE program, it takes the place of any traditional Medicare or HMO plan you may already have.
Pathways PACE participants agree to receive all of their healthcare services through the program, as outlined in their individualized care plan created by the Pathways PACE Interdisciplinary Team (IDT). This team includes your primary care provider, registered nurse, social worker, physical, occupational, and recreational therapists, dietitian, home care coordinator, personal care attendant, transportation driver, and the center director.
If you currently see a specialist who is part of the Pathways PACE network of contracted providers, you may be able to continue receiving care from that provider. When you join the Pathways PACE health plan, you’ll be assigned a primary care physician (PCP) who is part of the Pathways PACE Day Center’s IDT and experienced in caring for older adults.
Please note: If you receive services that are not authorized by Pathways PACE or not covered under your care plan, you may be personally responsible for those costs.
Pathways PACE provides transportation to and from the Day Center as an important part of its benefits. Transportation is available not only between your home and the Day Center but also to medical appointments with specialists and other activities included in your individualized care plan.
While at the Pathways PACE Day Center, participants can visit the on-site clinic for medical care, enjoy a nutritious lunch, take part in social activities, and receive various therapies—all before being safely transported back home at the end of the day.
No, focusing on just one part of care is not recommended. Seniors in the Pathways PACE program have the best chance to stay independent when their care addresses all aspects of their health. For example, after a stroke, a participant might need rehabilitation along with a special diet and medications for conditions like high blood pressure or diabetes. They may also experience feelings of loneliness or isolation. Pathways PACE tackles these challenges through a team of skilled healthcare professionals who create a comprehensive care plan that looks at the whole person’s health and well-being.
Not typically. On average, Pathways PACE participants visit the Day Center two to three times each week. How often a participant attends depends on the needs outlined in their individualized care plan and can range from once a week to several days a week, based on what is best for their health and well-being.
