The intake process begins when you, or someone on your behalf, contacts Pathways PACE. An Enrollment Specialist will visit with you to explain our program and obtain further information about you. During this visit:
The Pathways PACE Enrollment Specialist will discuss the various financial options available based on your financial situation. Options for payment include Medicare, Medicaid, or private pay.
Pathways PACE serves people 55 years of age and older who qualify for nursing home level-of-care according to the State of Colorado guidelines. An assessor must complete a functional assessment with you. The assessor will gather information on your functional status, nutrition risks, physical/mental health, and living arrangements.
Once your functional and financial eligibility is established, Pathways PACE will schedule an assessment at the Day Center. Additional assessments for enrollment may occur in the home. Members of the Pathways PACE Interdisciplinary Team (IDT) will meet with you individually to discuss your needs and goals. IDT members include the primary care provider, registered nurse, social worker, physical therapist, occupational therapist, recreational therapist, dietitian, home care coordinator, personal care attendant, transportation driver, and center director.
Pathways PACE encourages you to include a family member, caregiver, or other representative that will be involved in your current or future care needs at the site visit and home visits. Collaboration of care with you, your IDT, and your caregiver(s) if applicable, is essential to identify your care plan goals. Following the assessment process, the IDT will determine if the program can appropriately meet your needs and if you have met all the eligibility criteria.
The Pathways PACE representative will meet with you and if you wish, your family or caregiver(s), to review and sign the appropriate enrollment materials and answer any questions you may have. During this time, we will discuss:
A person qualifies if she or he:
Yes. If a person meets the income and assets limits to qualify for Medicaid, the program pays for a portion of the monthly Pathways PACE premium. Medicare pays for the rest. If a person does not qualify for Medicaid, he or she is responsible for the portion of the monthly premium Medicaid would pay. Pathways PACE staff can help determine a person’s Medicaid eligibility.
The Pathways PACE program is both Medicare providers and long-term care providers. By enrolling in the Pathways PACE program, you will no longer be enrolled in a traditional Medicare/HMO plan if you have one.
Pathways PACE participants agree to receive all health care services defined in a participant’s individualized care plan developed by the Pathways PACE program’s Interdisciplinary Team (IDT). Pathways PACE IDT members include the primary care provider, registered nurse, social worker, physical, occupational, and recreational therapists, dietitian, home care coordinator, personal care attendant, transportation driver, and center director.
If you have a specialist that is part of the Pathways PACE network of contracted providers, you may be able to keep your current specialty care physician. When a participant enrolls in a Pathways PACE health plan, she/he is assigned a primary care physician (PCP) who is a member of the Pathways PACE Day Center’s Interdisciplinary Team (IDT) skilled in treating the healthcare needs of seniors.
Pathways PACE participants may be fully and personally liable for the costs of unauthorized or out of Pathways PACE program agreement services.

Pathways PACE participants are free to disenroll from PACE at any time and resume their benefits in the traditional Medicare and Medicaid programs. You can enjoy PACE benefits until the end of the month in which you disenroll.
